Job summary

Constellation Health Services in Newark, NJ seeks a dynamic Hospice/Palliative Nurse Practitioner for per diem coverage. The NP provides comprehensive hospice services under physician supervision, conducting assessments, planning and coordinating care with patients and families in home and community settings.

Requirements include a registered nurse with an accredited NP program, current DEA license, CPR, state licensure, and hospice experience.

Qualifications

• Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
• Has a current DEA license.
• Current nursing licensure in State and CPR certification.
• Previous home care/hospice experience is required.
• Accreditation from an approved certifying body for advanced practice nursing as required by State.
• Certification in a specialist area preferred (e.g. Hospice & Palliative Nursing).
• Excellent observation, verbal and written communication, problem solving and mathematical skills.

Responsibilities

• Provides medical care and manages terminal illness under physician supervision.
• Performs history and physical examinations and documents findings.
• Develops individualized care plans and prescribes medications/treatments.
• Completes hospice face-to-face visit requirements for eligibility.
• Assists with hospice eligibility decisions and recertification with the hospice team.
• Coordinates care with hospice interdisciplinary team and other practitioners.
• Maintains accurate clinical notes and documentation.
• Educates patients/families on preventive care and end-of-life issues.
• Ensures safe environment and continuity of care with family/caregivers.
• Establishes relationships with staff and community referral sources.
• Complies with Medicare/Medicaid rules and HIPAA requirements.
