Job summary

Affinity Hospice Management LLC is seeking a Nurse Practitioner to deliver comprehensive hospice care under physician supervision in Knoxville, TN. The NP will evaluate patients, diagnose and manage health issues, and direct the plan of care while coordinating with the interdisciplinary team.

The role requires a Master's in Nursing/Nurse Practitioner, state NP licensure, CPR certification, and experience in hospice or palliative care.

Maintains current state NP licensure and CPR certification.
DEA license if applicable per state regulations.

Responsibilities

Provide comprehensive hospice medical care under physician supervision.
Conduct history, physicals, ordering/interpretation of tests, and document findings.
Develop and update individualized plans of care and manage medications/treatments.
Complete hospice face-to-face visits per regulations and support eligibility decisions.
Collaborate with hospice interdisciplinary team to coordinate patient care.
Communicate plan of care with patient, family, and other healthcare providers.
Educate families on care, symptoms, and end-of-life issues.
Maintain accurate clinical notes and documentation.
Ensure safe environment and continuity of care.
Advise and educate staff and community on hospice care protocols.
