Job summary

Affinity Hospice Management LLC is seeking a qualified Medical Social Worker (MSW) to implement hospice standards of care and provide psychosocial support to patients and families. You will collaborate with the interdisciplinary team, assess needs, and participate in care planning and crisis intervention.

The role requires an MSW from a CSWE-accredited program, strong communication skills, and a valid driver's license. Hospice experience is preferred; CPR certification is required.

Qualifications

• MSW from a CSWE-accredited program is required.
• Minimum one year of healthcare/hospice experience preferred.
• Strong verbal and written communication skills.
• Must be a licensed driver with a valid automobile.
• CPR certification required.

Responsibilities

• Assess psychosocial status of patients and families; report to the care team.
• Conduct social evaluations, plan interventions, and provide crisis support.
• Educate on culture, advance directives, and care planning.
• Participate in interdisciplinary group meetings and care plan development.
• Coordinate with physicians, nurses, and community resources for services.
• Maintain accurate clinical notes and communicate changes promptly.
