Job summary

Qualifications

• Master's in Social Work (MSW) from a CSWE-accredited program.
• Minimum of 1 year healthcare or hospice experience.
• Hospice care experience preferred.
• Strong verbal and written communication and organizational skills.
• Contingent upon satisfactory references and background checks.
• Prolonged standing or walking; visual/hearing acuity.
• Must be a licensed driver with a valid insured vehicle.
• CPR certification required (hands-on demonstration preferred).
• Covid-19 vaccination required.

Responsibilities

• Assess psychosocial status of patients and families/caregivers and communicate findings.
• Conduct social evaluations including family dynamics and caregiver abilities; plan interventions.
• Educate interdisciplinary team on cultural needs and patient preferences.
• Educate on advance directives and care planning.
• Participate in development of individualized plan of care; attend meetings.
• Assist in understanding social factors related to health problems and death/dying.
