Job summary

The Care Team Hospice is seeking a compassionate Medical Social Worker (MSW) to join our Akron-based team. The position focuses on emotional support, counseling, advocacy, and resource coordination for patients and families navigating the hospice journey.

As part of an interdisciplinary team, you will address psychosocial needs, assist with advance care planning, and connect families with community resources, with potential for the role to grow from part-time to full-time as census expands.

Qualifications

• Master's Degree in Social Work (MSW) preferred.
• Bachelor's Degree in Social Work (BSW) accepted with supervision as per state regulations.
• Experience in hospice, home health, healthcare, or case management preferred.
• Knowledge of psychosocial assessment, crisis intervention, grief counseling, and community resources.

Responsibilities

• Conduct comprehensive psychosocial assessments and develop individualized care plans.
• Provide emotional support, counseling, crisis intervention, and guidance to patients, families, and caregivers.
• Assist with advance care planning, resource coordination, and bereavement support.
• Collaborate with physicians, nurses, chaplains, and the inter-disciplinary team for coordinated care.
• Maintain accurate documentation of assessments and care coordination in the EMR (Homecare Homebase).
