Job summary

The Care Team Hospice is seeking a compassionate Medical Social Worker (MSW) to provide emotional support, counseling, advocacy, and resource coordination for patients and families navigating the hospice journey in Akron, OH. This part-time role offers opportunities to grow toward full-time status while working with an experienced interdisciplinary team.

Qualifications

• Master's Degree in Social Work (MSW) from an accredited program preferred.
• Bachelor's Degree in Social Work (BSW) accepted with required supervision in accordance with state regulations.
• Previous experience in hospice, home health, healthcare, or case management preferred.
• Knowledge of psychosocial assessment, crisis intervention, grief counseling, and community resources.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify patient and family needs while developing individualized plans of care.
• Provide emotional support, counseling, crisis intervention, and guidance to patients, families, and caregivers throughout the hospice journey.
• Assist with advance care planning, community resource coordination, financial assistance referrals, and bereavement support.
• Collaborate with physicians, nurses, chaplains, and the interdisciplinary team to ensure coordinated, patient-centered care.
• Maintain accurate, timely documentation of assessments, interventions, and care coordination activities within the electronic medical record (Homecare Homebase).
