Qualifications

• One year of LPN experience in an acute care setting is strongly recommended.
• Hospice care experience is desired.
• Valid driver’s license and auto insurance in your name are required.
• Ability to communicate effectively with patients, families, and care teams.

Responsibilities

• Provide skilled nursing care in patient homes as ordered by physician.
• Follow established order sets and care plans.
• Monitor patient conditions and report changes to RN supervisor.
• Counsel patients and families on disease progression and care plan.
• Complete clinical documentation per agency protocol and Medicare guidelines.
• Participate in on-call requirements and team meetings.
