Job summary

Cleveland Clinic Home Care and Hospice is seeking a Licensed Practical Nurse to deliver in-home care across the Greater Cleveland area, including Akron/Canton. You will visit approximately six patients daily, provide direct nursing care, administer medications, and educate families while coordinating with the care team.

Roles include start/end remotely with 4th weekend and holiday rotations, and require an active Ohio LPN license and BLS.

Qualifications

• Graduate from an accredited practical or vocational nursing program.
• Current Ohio license as a Licensed Practical Nurse.
• One year of clinical experience as a professional nurse in an acute setting.
• BLS certification through AHA or Red Cross.
• Valid Ohio driver's license and proof of automobile insurance.
• Knowledge of JCAHO and Medicare standards and regulations.

Responsibilities

• Visit and provide care to patients in their homes.
• Administer medications within scope of practice.
• Collect and document patient data and communicate with health care team.
• Coordinate discharge planning and transitions of care.
• Educate patients and families about care plans and hospice/homecare benefits.
• Maintain complete and up-to-date documentation and files.
