Job summary

Absolute Hospice, Inc. is seeking a compassionate Licensed Practical Nurse (LPN) for PRN Hospice care in Ohio. You will perform hands-on nursing visits, clinical management, and patient education in patients’ homes or residences, building trusting relationships with families and the care team.

The role emphasizes continuous assessment, education, and collaboration to ensure high-quality end-of-life care and comfort for patients and families in a respectful, supportive environment.

Qualifications

• Minimum 2 years of nursing experience.
• Strong clinical assessment and communication skills.
• Compassionate, patient, and flexible approach to care.
• Ability to lift/transfer patients as needed.
• Proficient with computer documentation.
• Valid driver’s license and one-year clean driving record.
• Weekend availability is a plus.

Responsibilities

• Provide routine nursing visits and direct patient care.
• Establish professional relationships with patients, families, and interdisciplinary care teams.
• Continually assess and evaluate patients' needs and conditions.
• Educate patients and caregivers about their conditions and care plans.
• Maintain accurate and timely documentation and charting.
