Job summary

Beaumont ASHN in Ohio seeks a Licensed Practical Nurse I (LPN) for Hospice to deliver skilled nursing at patients' homes, following physician orders and RN-directed care plans. You will educate patients and families and document care per Medicare guidelines.

Requirements include an active LPN license in the state, at least one year in acute care (Hospice preferred), a valid driver's license with insurance, and strong communication skills.

Qualifications

• Active LPN license in the state of employment.
• One year of acute care experience is strongly recommended; hospice experience is desirable.
• Valid driver's license and auto insurance in your name.
• Ability to communicate clearly with patients, families, physicians and colleagues.

Responsibilities

• Provides skilled nursing care in a patient's home as ordered by the physician/medical director.
• Follows order sets and goals established by RN.
• Observes and monitors patient conditions and communicates changes.
• Reconciles medications prescribed by the physician.
• Counsels patient and family on disease progression and care plans.
• Documents all treatments and education per agency and Medicare guidelines.
