Qualifications

• Active LPN license in the state of employment.
• One year of LPN experience in an acute care setting is strongly recommended.
• Hospice care experience is desired.

Responsibilities

• Provide skilled nursing care in a patient’s home as ordered by the attending physician/medical director.
• Follow order sets and goals made by RN.
• Observe and monitor patient conditions and notify RNCM of changes.
• Reconcile medications prescribed by the physician.
• Counsel patient and family on the disease progression and management.
• Document all aspects of treatment and education per agency protocols and Medicare guidelines.
• Participate in on-call requirements and case conferences.
• Attend mandatory trainings and agency meetings.
