Job summary

Sanford Health, Luverne Cottage Hospice, Luverne, MN, is seeking a compassionate LPN or MA to join the cottage hospice team. You will provide comfort-focused, patient-centered care to individuals and families in the home care setting.

The role involves working with an interdisciplinary team to support symptom management, emotional comfort, and dignity for every patient, with a shift from 6:00 to 6:00 on a rotating basis, including every third weekend and some holidays.

Qualifications

• Graduate or diploma from an accredited practical nursing program. LPN license in good standing.
• One year of professional nursing experience preferred.
• Current unencumbered LPN license or multi-state licensure.
• BLS for health care providers required.
• IV therapy/management may be required per ND requirements.

Responsibilities

• Provide comfort-focused, patient-centered care in home care settings.
• Collaborate with an interdisciplinary team to manage symptoms and dignity for patients.
• Assess patient data and apply nursing knowledge to ensure safety and appropriate care.
