Job summary

Trustbridge Hospice, a member of Empath Health, seeks a compassionate Hospice LPN to deliver patient-centered care in the Central Palm Beach County area. Work hours are 8:30am–5pm with weekend availability.

As an LPN, you will support patients and families, under RN supervision, with assessments, care planning, and education to ease advanced illness. Florida LPN license and BLS are required; IV course is a plus.

Qualifications

• Current Florida LPN license is required.
• Current Basic Life Support (BLS) certification.
• 30-hour IV course is a plus.
• Reliable transportation with valid Florida driver’s license.

Responsibilities

• Assess patient and family needs under RN and physician guidance.
• Coordinate and provide individualized care plans emphasizing comfort and dignity.
• Use monitoring, intervention, and teaching skills to improve quality of life.
• Provide emotional support to patients and families during overnight care.
• Ensure compassionate, personalized service for every patient and family.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to join our team and provide high-quality, patient-centered care to individuals receiving hospice care in the Central Palm Beach County area. You will work 8:30am - 5pm with weekend availability.

Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness.

What you’ll Do

As a Hospice LPN, you will play a vital role in supporting patients and families during some of the most critical moments in care.

• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs
• Coordinate and provide individualized care plans that prioritize comfort and dignity
• Utilize monitoring, intervention, and teaching skills to enhance quality of life
• Provide emotional support and reassurance to patients and families during overnight care
• Ensure exceptional, personalized service and a positive experience for every patient and family

What You’ll Need

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
