Empath Health’s Trustbridge Hospice is seeking a dedicated Licensed Practical Nurse to provide Continuous Care visits across Palm Beach County. You will work with the RN and physician to assess needs, plan comfort-focused care, monitor and teach families, and provide overnight support with empathy.

Florida LPN license, BLS, and IV course training are required or preferred, with strong emphasis on compassionate, personalized Full Life Care for All. Excellent benefits and PTO accompany the role.

Qualifications

• Current license to practice as an LPN in Florida.
• Current Basic Life Support Certification.
• 30-hour IV course meeting Florida Board of Nursing requirements is a plus.

Responsibilities

• Work under the direction of the Registered Nurse and primary physician to assess patient and family needs.
• Coordinate and provide individualized care plans that prioritize comfort and dignity.
• Utilize monitoring, intervention, and teaching skills to enhance quality of life.
• Provide emotional support and reassurance to patients and families during overnight care.
• Ensure exceptional, personalized service and a positive experience for every patient and family.

Trustbridge Hospice, a member of Empath Health, is seeking a caring and motivated Licensed Practical Nurse (LPN) to provide Continuous Care hospice visits throughout Palm Beach County.
