Job summary

Trustbridge Hospice, a member of Empath Health, seeks a dedicated Licensed Practical Nurse (LPN) to provide continuous home hospice visits across Palm Beach County. Shifts run 7:00 a.m. to 7:30 p.m. with every other weekend coverage, delivering compassionate, high-quality care in patients' homes.

You’ll work under a Registered Nurse and physician to tailor care plans, monitor comfort, and support families through critical moments, ensuring dignity and peace.

Qualifications

• Current license to practice as a Licensed Practical Nurse in the State of Florida.
• Current Basic Life Support Certification.
• Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.

• Coordinate and provide individualized care plans that prioritize comfort and dignity.
• Utilize monitoring, intervention, and teaching skills to enhance quality of life.

This role offers the opportunity to support patients and families during critical moments by delivering compassionate, high-quality care in the comfort of their homes on the following schedule: 7am - 7:30pm and every other weekend. Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care.
