Job summary

Calvary Hospital seeks a referrals coordinator for Calvary Hospice in the New York area. You will manage incoming hospice referrals, coordinate with clinical staff, and ensure accurate and timely admission processes.

Responsibilities include documenting referral details in the EMR, verifying insurance coverage (Medicare/Medicaid and managed care), and communicating coverage limitations to families and leadership.

Qualifications

• Minimum of four years experience in a coordinator or related capacity.
• Experience in a Hospice, Home Health Agency or Hospital is preferred.
• Proficiency with Word and Excel; familiarity with medical terminology.
• Experience in intake and customer service activities is preferred.
• Excellent telephone etiquette required.
• Bi-lingual language skills preferred.
• Ability to maintain confidentiality and highly developed organizational skills.
• Exhibits an attitude of caring and Calvary Hospital spirit.

Responsibilities

• Serve as the primary point of contact for incoming hospice referrals from hospitals, physicians, facilities, and community sources.
• Collect and document complete referral and demographic information; initiate contact with patients/families to explain the hospice admission process.
• Coordinate with clinical staff to facilitate eligibility review and admission, escalating questions to RN as needed.
• Verify insurance coverage and benefits (Medicare, Medicaid, managed care, commercial plans) and obtain pre-authorizations when required.
• Collaborate with Finance to ensure timely and accurate insurance verification and documentation.
• Explain hospice benefits and required documentation to patients and families; communicate coverage limitations to leadership.
• Maintain real-time documentation of referral status, pending admissions, and authorization updates in the EMR.
• Monitor and follow up on pending referrals to ensure timely progression to admission.
• Coordinate and schedule hospice admissions.
