Job summary

VNS Health is seeking a Hospice Liaison in Brooklyn, NY to help patients transition from hospital to hospice. You will evaluate referrals, coordinate admissions, and provide counseling to families and referrers for end-of-life care options.

The role requires strong clinical skills, a valid NY RN license, and excellent communication. Responsibilities include coordinating initial hospice services, ensuring equipment and medications are in place, and collaborating with the Hospice Team for seamless

Qualifications

• Licensed Registered Nurse in NY
• Minimum two years clinical experience, hospice/palliative care preferred
• Strong communication and customer service skills
• Ability to negotiate and coordinate care transitions

Responsibilities

• Evaluate referrals for hospice admission and coordinate plans of care
• Conduct in-facility assessments and admissions when appropriate
• Provide counseling to referrers, patients, and families
• Serve as liaison during discharge and initiation of Hospice Team services
• Coordinate initial hospice services with Hospice Team Coordinator and ensure equipment/services are in place
• Collaborate with clinical staff to meet patient needs at discharge or home hospice
• Build relationships with program staff and facilities for referrals
• Determine insurance coverage and communicate to families and hospital staff
• Participate in Continuous Quality Improvement meetings and report findings
• Represent VNS Hospice at community programs and events
