Job summary

Legacy Hospice, Inc. is seeking an experienced RN to serve as a Clinical Liaison, bridging patient care and business development in hospital and care-transition settings.

You will educate stakeholders, assess eligibility, coordinate referrals, and support timely transitions to hospice services while building strong partnerships with physicians, discharge planners, and care teams. The role requires RN licensure, hospice experience, and the ability to work independently in a fast-paced

Qualifications

• RN license in state of practice
• Graduate of an accredited school of nursing
• Valid driver’s license and reliable transportation
• Hospice experience required
• Ability to work independently and manage multiple priorities

Responsibilities

• Represent Legacy Hospice in hospitals and transitional care settings.
• Educate stakeholders about hospice benefits and services.
• Assess eligibility and coordinate referrals with branch teams.
• Build relationships with physicians and discharge planners.
• Ensure documentation in the EMR (HCHB) is accurate and timely.
• Support program growth through market analysis and strategy input.
