Job summary

VNS Health in New York seeks an experienced Registered Nurse for Hospice care to join our team. You will assess patient and family needs, develop and revise care plans, and deliver direct nursing services in patients’ homes or facilities under supervision.

Qualifications

• Licensed Registered Nurse in New York State required
• Associate's Degree in Nursing; Bachelor's degree preferred
• Minimum of one year of clinical nursing experience including Hospice, Med/Surg, ICU, ED, Home Care, Palliative Care, Oncology or LTC

Responsibilities

• Assesses patient/family needs through observation, interview and exam, and updates care plan as needed.
• Coordinates and evaluates the care plan with the interdisciplinary team.
• Delivers direct nursing care in patients' homes or facilities, using approved equipment.
• Travels to patients' homes or facilities with equipment and supplies (approx. 25–30 lbs).
• Maintains documentation and communicates with physicians and team about changes in patient needs.
• Participates in interdisciplinary team meetings and patient/family counseling, including bereavement support.

Education

Associate's Degree in Nursing

Bachelor's Degree in a health or human services field or Nursing from a State-approved diploma program
