Job summary

Affinity Hospice Management LLC is seeking an Intake Manager to oversee all aspects of patient intake, lead the intake team, and build strong relationships with referral sources and customers. You will supervise processes for admissions, insurance verification, and authorizations to ensure accurate and timely care transitions.

The role requires at least three years in healthcare, preferably in home care operations, with strong communication, negotiation, and organizational skills.

Qualifications

Responsibilities

• Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
• Enters referral/patient information into electronic medical record system and other applications as needed.
• Coordinates with referral sources, marketing and clinical teams to ensure patient transition into hospice care.
• Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
• Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
• Establishes and maintains positive working relationships with current and potential referral sources.
• Ensures direct oversight of insurance verification and authorization processes.
• Maintains comprehensive working knowledge of Continuum Care’s contractual relationships and ensures that patients are admitted according to contract provisions.
• Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Continuum Care.

JOB DESCRIPTION SUMMARY

Responsible for managing all aspects of the patient intake process including managing the members of the intake team, establishing and maintaining positive relationships with

customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

POSITION QUALIFICATIONS

1. High School Diploma required/ College Degree preferred.
2. Proficient in Google Drive/Sheets/Docs preferred.
3. Minimum of Three (3) years’ experience in health care, preferably in home care operations.
4. Demonstrated ability to supervise and direct professional and administrative personnel.
5. Ability to market aggressively and deal tactfully with customers and the community.
6. Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.
7. Demonstrates good communications, negotiation, and public relations skills.
8. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
