Job summary

Affinity Care of Sarasota is seeking an experienced Intake Manager to oversee the patient intake process, lead the intake team, and foster strong relationships with customers and referral sources. You will coordinate insurance verification and authorization workflows to ensure timely admissions and accurate record-keeping.

Qualifications

• High School Diploma required; College Degree preferred.
• Minimum of three (3) years’ experience in health care, preferably in home care operations.
• Demonstrated ability to supervise and direct professional and administrative personnel.
• Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.
• Demonstrates good communications, negotiation, and public relations skills.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Responsibilities

• Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
• Enters referral/patient information into electronic medical record system and other applications as needed.
• Coordinates with referral sources, marketing and clinical teams to ensure patient transition into hospice care.
• Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
• Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
• Establishes and maintains positive working relationships with current and potential referral sources.
• Ensures direct oversight of insurance verification and authorization processes.
• Maintains comprehensive working knowledge of Continuum Care’s contractual relationships and ensures that patients are admitted according to contract provisions.
• Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Continuum Care.
