Heart to Heart Hospice is seeking an Intake Coordinator to manage daily patient referrals and intake operations. You will initiate charts in the EMR, maintain workflow, and ensure compliance with CHAP and state/federal requirements. Build positive relationships with referral sources to support high-quality care.

Qualifications

• It lists a degree in health care or related field as preferred.
• Experience in admissions/intake and customer service is required.

Responsibilities

• Direct all daily patient referral and intake operations.
• Initiate chart in EMR and maintain workflow.
• Ensure compliance with state, federal and CHAP referral requirements.
• Maintain relationships with referral sources to enhance perceptions of Heart to Heart Hospice.
• Gather and report referral statistics and trends.
• Participate in strategic planning for better services.
• Submit all documentation timely and accurately.
