Job summary

Hospice Services of Massachusetts is seeking a Clinical Intake Coordinator to join our Lakeville office in-person. This full-time role pays $37-$40 per hour, with weekly pay, and requires an active LPN or RN license in Massachusetts.

You will oversee the patient intake process, maintain relationships with referral sources, respond to requests and concerns, and ensure timely admission of patients in compliance with regulations. You may be required to visit patients as needed.

Qualifications

• Must possess an active and unencumbered LPN or RN license in Massachusetts.
• Prior hospice or home health experience preferred.

Responsibilities

• Receives and accepts all daily patient referrals through phone, fax, and electronic solutions.
• Ensures compliance with all state and regulatory requirements with information received.
• Use work methods and procedures that ensure patients are admitted in accordance with policy.
• Establishes and maintains positive working relationships with current and potential referral sources.
• Builds and monitors community and customer perceptions of the agency as a high-quality provider of services.
• Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources.
• Documents physician orders for intake/referrals accepted by all intake coordinators as well as own.
• Assures timely completion of all documentation to assure that patients are admitted timely.
• Follows up with referral source to check status of pending referrals.
• Must be willing to make patient visits, as needed.
