Job summary

Hospice of Western & Central Massachusetts is hiring a full‑time Admissions & Intake Coordinator based out of Agawam, MA, with extensive travel across western and central Massachusetts. Travel and a reliable insured vehicle are required, with mileage reimbursed.

The role is salaried with weekly pay up to $48 per hour. The coordinator will screen admissions, coordinate referrals, and ensure timely admission of eligible patients to the agency.

Qualifications

• Active Massachusetts RN license required.
• Hospice experience preferred.
• At least 1 year of nursing experience required.
• Experience with EMR systems preferred.

Responsibilities

• Receive, review, and evaluate hospice referrals from hospitals, facilities, physicians, and families.
• Gather and review clinical documentation to evaluate hospice eligibility.
• Communicate with referral sources to obtain missing documentation for timely evaluation and admission.
• Coordinate with Medical Director and clinical leadership on eligibility and complex referrals.
• Oversee referral process from inquiry through admission with timely communication to patients and facilities.
• Perform comprehensive nursing assessments to determine hospice eligibility.
