Qualifications

• LPNs with home health intake experience.

• Ability to develop relationships, plan and coordinate care and resources.
• Ability to work with minimal supervision and adapt to changing priorities.

Responsibilities

• Acts as liaison with referral sources, health professionals, and field staff.
• Receives and coordinates incoming calls; provides information to patients/clients and sources.
• Communicates with patients/families about initiation of care/services.
• Performs intake screening and creates preliminary admission decisions.
• Maintains patient information in the management system and creates files.
• Assists with scheduling and coordination of field staff and appointments.
• Ensures compliance with standards of nursing practice and applicable regulations.
