Qualifications

• Graduate of an accredited school of practical nursing.
• Completion of the Agency’s Orientation Program of the Valley Home Care.
• Driver's License – must have an acceptable driving record.

Responsibilities

• Support the referral, screening, and onboarding process for hospice patients.
• Assist in reviewing incoming referrals and clinical information.
• Coordinate with physicians, referral sources, patients, and families for admissions.

Skills

Current and valid NJ license and CPR certification

Critical thinking skills

Effective interpersonal and communication skills

Current and valid driver's license

Education
