Job summary

Affinity Health Management is seeking an Intake Supervisor to manage all aspects of the patient intake process, including overseeing the intake team, building relationships with referral sources, and handling insurance verification and authorizations.

This role requires experience in health care, strong communication, and knowledge of CHAP standards and payer practices; bilingual preferred.

Qualifications

• High School Diploma required; college degree preferred.
• Minimum of three years’ experience in health care, preferably in home care operations.
• Demonstrated ability to supervise and direct professional and administrative personnel.
• Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practices.
• Demonstrates good communications, negotiation, and public relations skills.
• Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.

Responsibilities

• Directs all daily patient referral and intake operations including establishment and implementation of intake policies.
• Enters referral/patient information into electronic medical record system and other applications as needed.
• Coordinates with referral sources, marketing and clinical teams to ensure patient transition into hospice care.
• Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
• Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy.
• Establishes and maintains positive working relationships with current and potential referral sources.
• Ensures direct oversight of insurance verification and authorization processes.
• Maintains comprehensive working knowledge of Continuum Care’s contractual relationships and ensures patients are admitted according to contract provisions.
• Maintains knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Continuum Care.

JOB DESCRIPTION SUMMARY

customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.

POSITION QUALIFICATIONS
