We are looking for a Full Time Intake Coordinator to join our team! The Intake Coordinator is responsible for managing all aspects of the patient intake process including working alongside the members of the Clinical and Marketing teams, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.
Essential Functions/Responsibilities:
- Answer incoming calls and direct all daily patient referral and intake operations.
- Abides by all state, federal, and ACHC referral/intake regulatory requirements.
- Acts in accordance with work methods and procedures to ensure patients are admitted in accordance with policy.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Ensures seamless transition of patients to Hospice through collaboration with the internal team and if needed multiple service providers.
- Ensures maximum third-party reimbursement through direct oversight of insurance verification and authorization processes which includes Medicare/Medicaid/Private Insurance.
- Builds and monitors community and customer perceptions of the organization as a high quality provider of services.
- Responsible for obtaining admission documents such as, but not limited to consents, POLST, clinical documentation, insurance and transaction forms.
- Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted according to contract provisions.
- May participate in quality assessment performance improvement teams and activities.
- Responsible for completing the referral process in the EMR system for all admissions.
- Responsible for collecting and depositing payments for In-Patient Unit in a timely manner.
- Responsible for monitoring the referral list and coordinating with internal staff for admissions.
- Responsible for building new physicians into EMR and verifying License/OIG/SAMS verifications to meet compliance requirements at onset and yearly.
- Participates in weekly, weekend, holiday on-call coverage as necessary.
- Other duties as assigned by the Administrator.
Qualified applicants will possess the following:
- High school diploma or equivalent required.
- Complete Hospice Orientation and Hospice 101.
- Ability to work independently, be organized and detail oriented.
- Ability to develop and maintain positive working relationships with staff and community partners and referral sources.
- Ability to communicate effectively verbally and in writing.
- Knowledge of hospice services and rules and regulations.
- Acceptance of and adaptability to different social, racial, cultural and religious modes.
- One (1) year experience in Intake preferred.
- Three to five (3-5) years of health care experience, preferred.
