Job summary

At Home Quality Care is seeking a Hospice Referral/Intake Specialist to manage patient referrals and intake while coordinating across multiple service providers. You will verify insurance, authorize processes, and maintain strong relationships with referral sources and customers.

The role requires at least three years in health care, preferably hospice, with solid communication skills and the ability to navigate CHAP standards and private payer practices.

Qualifications

• High School diploma or equivalent.
• At least three (3) years’ experience in health care, preferably in hospice or palliative operations.
• Demonstrated ability to supervise and direct professional and administrative personnel.
• Ability to deal tactfully with customers and the community.
• Knowledge of corporate business management, governmental regulations, CHAP standards, and private payer practice preferred.
• Demonstrates good communication skills.
• Demonstrates autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities.

Responsibilities

• Performs all daily patient referral and intake operations.
• Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
• Establishes and maintains positive working relationships with current and potential referral sources.
• Ensures seamless transition of patients to hospice by coordinating care with multiple service providers.
• Ensures maximum third-party reimbursement by conducting the insurance verification and authorization processes.
• Builds and monitors community and customer perceptions of At Home Quality Care as a high-quality provider of services.
• Gathers, collates, and reports referral statistics including key customer referral trends.
• Maintains comprehensive working knowledge of contractual relationships and ensures patients are admitted according to contract provisions.
• Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided.

Job Description Summary

Responsible for performing all aspects of the patient intake process, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and conducting the insurance verification and authorization process.
