Job summary

JourneyCare Hospice in Crystal Lake, IL is seeking an Intake Coordinator to join our growing care team. You will receive referrals, explain benefits to patients and families, and help arrange admissions and initial nursing assessments.

The role emphasizes accurate documentation, strong communication, and coordinating with admissions staff; hours are Mon-Fri 9:00am-5:30pm with every 4th weekend. We offer comprehensive benefits, a supportive culture, and opportunities for growth.

Qualifications

• Must be 18 years or older.
• At least 1 year experience in homecare, hospice, or healthcare field preferred.
• Ability to complete necessary documentation accurately, legibly, and timely.
• Strong interpersonal, verbal and written communication skills.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Receive referrals by phone or in person for designated department.
• Provide an explanation of the benefits to patients, families, and community contacts.
• Assist with finding alternative services when the patient is not a candidate for hospice.
• Oversee scheduling of initial nursing assessment and admission.
• Maintain admission packets and ensure proper forms are available to admissions staff.
• Assist community outreach representatives with marketing activities to promote relations with referral sources.
• Assist with the completion of admission paperwork as needed.
• Adhere to work schedule and maintain punctuality and attendance.
• Provide telephone and clerical support to other staff members as time allows.
• Maintain accurate clinical records, tracking goals and progress of clients.

• Ability to complete necessary documentation appropriately, legibly, and timely
• Ability to handle stressful and delicate situations
• Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
• Valid driver’s license and proof of insurance are required

Hours: Monday-Friday9:00am-5:30pm - Plus Working Every 4th Weekend

Location: Crystal Lake, IL.

Pay Range: $20-$28 hourly - offer based on experience.
