Job summary

JourneyCare Hospice in Lake Forest, IL is seeking a Hospice Social Worker to assess psychosocial needs and support patients and families within the hospice program. The role emphasizes timely care planning, collaboration with the care team, and community resource coordination.

Eligible candidates will hold an MSW or equivalent, possess current state licensure in Social Work, and have at least one year of healthcare or hospice experience.

Qualifications

• MSW required; Social Work degree from accredited program.
• Current State Licensure in Social Work.
• Minimum of one year of experience in health care or hospice settings.
• Valid driver's license with proof of insurance.

Responsibilities

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care.
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided

• Provides accurate and timely documentation of patient services to reflect the plan of care

Salary Range

$62,000-$80,000 - Offer based on experience
