Seasons Hospice in Springfield, MO is seeking an Intake Coordinator to lead the front door of the agency, coordinating referrals, admissions, and payer communications with precision and heart.

In this role you will partner with hospitals and physicians, ensure timely patient admissions, and support seamless transitions from referral to care, while upholding Seasons Hospice’s mission of compassionate, compliant service to patients and families.

Qualifications

• 3–5+ years of healthcare experience, hospice or home health preferred.
• Strong leadership, organization, and problem-solving skills.
• Experience with intake, admissions, or referral coordination.
• Ability to build relationships and represent Seasons Hospice in the community.
• Knowledge of insurance processes, authorizations, and healthcare regulations.
• Clear, professional communication and compassionate presence.
• Detail-oriented with a big-picture perspective.

Responsibilities

• Oversee and coordinate all patient referral and intake operations.
• Build and maintain relationships with hospitals, physicians, and community partners.
• Ensure patients are admitted quickly, accurately, and in compliance with regulations.
• Manage insurance verification, authorizations, and payer communication.
• Collaborate with clinical teams to ensure seamless care transitions.
• Track referral trends and contribute to growth initiatives.
• Support quality improvement and enhance patient experience.
• Serve as a trusted resource for patients, families, and referral sources.
