Job summary

Swan Hospice in the Lakewood area seeks an Office Intake Coordinator to manage incoming patient referrals, coordinate admissions, and ensure timely documentation to support smooth patient care initiation.

You will serve as the primary contact for patients, families, hospitals, and referral sources, handling eligibility screening, data entry into EHR systems (MatrixCare), and communications about services and admission steps.

Qualifications

• High School Diploma required; associate/bachelor's preferred.
• Experience in healthcare intake, hospice, home health, or case management.
• Strong organizational, multitasking, and communication skills.
• Proficiency in Microsoft Office and EHR software.

Responsibilities

• Receive and process new patient referrals from hospitals, physicians, and other providers.
• Conduct initial eligibility screening for hospice services.
• Schedule admissions visits with clinical teams.
• Collect and verify patient demographic, insurance, and clinical information.
• Enter referral and patient data into EHR or MatrixCare.
• Communicate with families about hospice services, benefits, and admission steps.
• Coordinate with intake and clinical staff for timely admissions.
• Maintain tracking logs and reports for referrals and admissions.
• Support billing and compliance with required documents before start of care.
• Provide exceptional customer service and professional communication with stakeholders.
