Qualifications

• Customer service experience of at least 1 year.
• Proficiency in medical terminology with at least 1 year.
• Strong computer skills including EHR navigation and MS Office.

Responsibilities

• Review patient information to determine hospice eligibility.
• Coordinate with clinicians, patients, and families to arrange care.
• Document patient data accurately and manage intake paperwork.
• Explain hospice services and obtain necessary consents.
• Coordinate referrals and transitions to hospice care.
