Continuum Care Hospice seeks a qualified Nurse Practitioner to provide comprehensive hospice services under physician supervision in patients' homes and communities.

The NP will perform history and physicals, assess eligibility for hospice, develop and adjust plans of care, prescribe medications, and coordinate with the hospice team, patients, and families to ensure high-quality, compassionate care in alignment with state Nurse Practice Act.

Qualifications

• Requires NP license and valid Florida practicing license.
• Provides hospice care under physician supervision.
• Experience in palliative/hospice care preferred.

Responsabilités

• Provide comprehensive hospice assessment and care.
• Develop and manage patient care plans and symptom management.
• Coordinate with inter-disciplinary team, patients and families.
• Document medical records and ensure compliance with regulations.
