Job summary

Blue-Summit Hospice and Palliative Care in Roswell, GA is seeking a qualified Nurse Practitioner to provide specialized hospice care to terminally ill patients in home or facility settings. The NP will work as part of the Hospice team to deliver compassionate, patient-centered care and support families through bereavement.

This role requires RN Practitioner licensure in the state, ability to work independently, supervise others, handle crisis situations, and participate in hospice training.

Qualifications

• Graduate from an accredited nursing program.
• Licensed as a Nurse Practitioner in the state.
• Minimum 2 years of experience as a Nurse Practitioner, preferred.
• Completion of Hospice training program.

Responsibilities

• Regularly assesses and reassesses the Hospice patient needs.
• Orders, performs, supervises and interprets diagnostic tests.
• Initiates and manages treatment plans, including medications.
• Teaches, supervises, educates and counsels the Hospice patient and family.
• Supervises and trains other nursing service personnel.
• Develops and re-evaluates the patient/family care plan with IDG.
• Maintains records and adheres to HIPAA policies.
• Participates in team conferences and on-call coverage.
