Job summary

Blue Summit Hospice in Roswell, GA is seeking a Nurse Practitioner to provide specialized hospice medical care to terminally ill patients, in home and facility settings, with support from the Hospice team. The NP will work under supervision of the Medical Director/Director of Patient Services, assess patients, order tests, manage treatments, and counsel families through bereavement.

Candidates should be licensed NP in Georgia, with strong independent practice and crisis response skills;

Qualifications

• Graduate from an accredited school of nursing.
• Current state NP licensure to practice in Georgia.
• Ability to work independently and respond to crises.

Responsibilities

• Assess and reassess Hospice patient needs.
• Order, perform, supervise, and interpret diagnostic tests.
• Initiate and manage treatment plans, including medications.
• Educate patient and family about care and bereavement support.
• Supervise and train nursing staff involved in Hospice care.
• Coordinate care plans with the IDG for continuity.
• Maintain patient records per Hospice policies.
• Participate in on-call coverage and team conferences.
