Job summary

Stillwater Hospice is seeking an RN Case Manager to provide in-home nursing care across Kosciusko, Noble, Whitley, and DeKalb counties. You will assess needs, develop and revise care plans, and coordinate the interdisciplinary team to ensure compassionate, quality hospice care.

The ideal candidate holds an Indiana RN license, CPR certification, and preferably a BSN with 1 year of home health or hospital experience. Travel to patient homes is required and strong communication is essential.

Qualifications

• Graduate of an accredited nursing program.
• Bachelor’s degree in nursing preferred.
• Current RN license to practice in Indiana.
• CPR certification current or to be obtained within orientation period.
• Prefer one year of recent hospital or home health care experience.

Responsibilities

• Care for patients in their homes and coordinate nursing care.
• Assess patient needs, implement and adjust nursing care plans, evaluate outcomes.
• Prepare clinical and progress notes and conduct initial assessments (including OASIS where applicable).
• Supervise LPNs, Home Health Aides, CNAs, and Homemakers as needed.
• Participate in interdisciplinary care conferences and maintain professional boundaries.
