Job summary

Heart to Heart Hospice is seeking an experienced RN Case Manager in Indiana. The role requires clinical expertise in hospice care, strong assessment skills, and the ability to coordinate with a multidisciplinary team to deliver patient-centered plans of care.

As a full-time employee, you will conduct comprehensive assessments, implement and revise care plans, attend IDG meetings, and provide on-call support while ensuring timely documentation and regulatory compliance.

Qualifications

• Must be a graduate of an accredited nursing program and hold an active RN license.
• Valid driver’s license, reliable transportation, and ongoing auto liability insurance.
• Two years RN experience preferred in healthcare/clinical setting.

Responsibilities

• Initial and ongoing assessments of the impact of the terminal disease on the patient’s physical, psychosocial, and environmental needs and ADLs.
• Implement the individualized Plan of Care and revise the plan as necessary.
• Collaborate with patient, caregiver, physicians, and healthcare professionals.
• Attend Inter-Disciplinary Group (IDG) meetings and coordinate patient care planning; prioritize needs and promote care coordination.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Perform other duties as required; ensure compliance with laws and policies.
