Responsibilities

• Follows written care instructions from RN or Therapist.
• Assists patients with personal care and transfers.
• Performs simple therapy-related procedures (ambulation, ROM, exercises).
• Assists in medication administration under physician/nurse direction as allowed by state.
• Reports changes in patient condition to supervisor or RN.
• Maintains records of services and patient condition.
• Establishes good relationships with patient/family and co-workers.
• Participates in patient care conferences for continuum of care.
• Performs household services essential to home care.
