Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to provide compassionate, person-centered nursing care in patients’ homes. You will assess patients, implement the nursing process, and work with the Interdisciplinary Group (IDG) to plan and direct individualized care plans.

The role emphasizes clinical expertise, dignity and comfort at end-of-life, and collaboration with IDG to meet patient and family needs across the home environment.

Qualifications

• Associate or Bachelor’s degree from an accredited School of Nursing.
• Current, unencumbered RN license in the state of employment.
• BLS certification within 30 days of hire.

Responsibilities

• Complete patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Write and initiate the Plan of Care in collaboration with the IDG.
• Regularly reevaluate needs and revise the Plan of Care as necessary.
• Develop care plans with patient, family, caregiver, and IDG.
• May administer medications as prescribed by the IDG plan.
• Participate in on‑call duties as defined by procedures.
• Maintain productivity as per organizational guidelines.
