Job summary

Virtua Health is seeking a Hospice Registered Nurse to bring hospice care into patients' homes across New Jersey, including Marlton. The RN will assess, plan, and implement a care plan to keep terminally ill patients comfortable and supported through this transition.

Collaboration with the Interdisciplinary team is essential to coordinate patient care and educate families. Responsibilities include administering medications, monitoring symptoms, documenting care, and participating in on-call

Qualifications

• Minimum of 2-3 years of nursing experience required; 2 years hospice experience preferred.
• Knowledge of physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers.
• Graduate of an accredited nursing program; BSN preferred.

Responsibilities

• Perform and document nursing process – assessment, diagnosis, planning, implementation, and evaluation of patients receiving care in-person at their defined home.
• Provide nursing care, emotional support and education to patients and families.
• Administer medications and treatments as written in the plan of care to obtain comfort and relieve symptoms.
• Observe and report signs and changes in patient condition to the Physician and team.
