Job summary

Virtua Health is seeking a Hospice Registered Nurse (per diem, part-time) to deliver patient-centered care in patients' homes across the service area. The RN will perform assessments, coordinate plans, manage symptoms, and support families through end-of-life care with a team-based approach.

The role requires traveling statewide to provide compassionate relief and comfort, working in a multi-disciplinary setting while maintaining high professional standards.

Qualifications

• Perform nursing process - assessment, diagnosis, planning, implementation, and evaluation.
• Provide nursing care, emotional support and education to patients and families.
• Administer medications and treatments per plan of care to maintain comfort.
• Observe and report signs of disease progression and patient condition changes.
• Collaborate with the Interdisciplinary team to coordinate care.

Responsibilities

• Perform and document nursing assessments and create care plans for home-based hospice patients.
• Provide compassionate nursing care and education to patients and families.
• Administer medications and treatments per the patient’s plan of care.
• Monitor and report changes in patient condition to the physician and team.
• Support families through the hospice process and coordinate with other providers.
