Job summary

Virtua Health is seeking a Hospice Registered Nurse Case Manager to deliver compassionate end‑of‑life care in patients’ homes across New Jersey. You will assess needs, develop care plans, and coordinate with physicians and the interdisciplinary team to ensure comfort and quality of life.

The role requires 2–3 years hospice nursing experience, NJ RN licensure, and flexibility for on‑call, weekend and holiday coverage. Comprehensive benefits accompany full‑time employment.

Qualifications

• Minimum of 2-3 years hospice nursing experience.
• Current State of New Jersey nursing license to practice as an RN.
• CPR/BLS certifications preferred.

Responsibilities

• Provide compassionate hospice nursing care in patients' homes within the Virtua Hospice service area of New Jersey.
• Coordinate care with physicians and interdisciplinary team; order meds, supplies, and DME.
• Participate in on-call, weekend, and holiday coverage as needed.
