Job summary

Virtua Health in New Jersey is seeking a Hospice Registered Nurse to bring the hospice care model into patients’ homes, assessing, planning, and implementing treatment plans to keep terminally ill patients comfortable.

The RN will monitor vital signs, administer medications, and support patients and families through the transition, working within a team to coordinate care and maintain high ethical standards. Travel statewide is required.

Qualifications

• Must hold active New Jersey nursing license.
• Minimum of 2–3 years nursing experience; hospice/home care preferred.
• Excellent assessment skills and clinical judgement.

Responsibilities

• Perform and document nursing process in patients' homes.
• Follow up on family education and care plans.
• Triage changes in patient condition and respond accordingly.
• Provide nursing care, emotional support, and patient/family education.
• Administer medications per plan of care to relieve symptoms.
• Communicate findings to the physician and care team.
• Coordinate care with interdisciplinary team and community resources.
• Participate in on-call coverage, weekends, and holidays.
