Qualifications

• RN license in good standing.

• Excellent observation, verbal and written communication skills; problem solving and math skills.

Responsibilities

• Perform comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Create and revise the plan of care with patient and family involvement.
• Administer medications and treatments as prescribed by the physician.
• Educate and counsel patients and families; coordinate care with the interdisciplinary team.
• Participate in on-call duties and ensure equipment and services are available.
