Job summary

Affinity Care of Sarasota is seeking a Registered Nurse to plan, organi ze, and direct hospice care in home and community settings. The nurse will assess patients and families, develop plans of care, and coordinate interdisciplinary team efforts to meet patient needs.

You will administer medications, educate families, and ensure a safe care environment while supporting ongoing quality improvements and case management responsibilities.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse, with a License in good standing to practice in the state of Florida.
• Minimum of 1 year of experience as a professional nurse within the last three years; or a BSN with at least two years’ experience (preferably in public health, home care, or hospice).
• Management experience not required. Responsible for supervising hospice aides.
• Satisfactory references and criminal background checks required by regulation.
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
• Must be a licensed driver with insured vehicle in good working order.
• CPR certification (hands-on demonstration if required).

Responsibilities

• Completes comprehensive assessments of patient and family to determine hospice needs and develops a plan of care.
• Provides professional nursing care utilizing the nursing process.
• Writes and initiates plan of care; regularly re-evaluates patient and family needs.
• Initiates revisions to the plan of care as needed based on patient status.
• Administers medications and treatments as prescribed.
• Counsels patient and family regarding nursing and related needs.
• Provides health care instructions and supports activities of daily living.
• Acts as Case Manager and coordinates patient care for assigned caseload.
