Affinity Care of Sarasota is seeking a qualified Registered Nurse to lead hospice care delivery in patients’ homes across the Sarasota area. The role emphasizes community health education and direct patient care within home settings, coordinating with families and interdisciplinary teams.

The ideal candidate will have Florida RN licensure, current CPR, and at least one year of recent hospice/home health experience.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed Registered Nurse in good standing in Florida.
• 1 year of professional nursing experience in the last 3 years (or a BSN with 2 years’ experience in public health, home care, or hospice).
• Management experience not required. Supervisory duties for hospice aides only.
• Background checks required upon offer.
• Excellent observation, verbal and written communication skills; basic math ability.
• Must have a valid driver’s license with insured vehicle.
• CPR certification (hands-on preferred).

Responsabilités

• Complete initial, comprehensive, and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the care plan with patient/family involvement as needs change.
• Administer medications and treatments per physician’s plan of care.
• Counsel patient and family on nursing needs and self-care techniques.
• Coordinate patient care as Case Manager when assigned.
• Maintain accurate clinical notes and report changes in patient condition.
