Affinity Care of Sarasota is seeking a licensed Registered Nurse to lead hospice care in home settings. The role emphasizes community health education and direct patient/family support within their homes.

Formación

• RN licensed and in good standing to practice.
• Graduate of an accredited school of nursing.
• Experience in home health, hospice, or public health preferred.
• Excellent observation, communication and problem-solving skills.

Responsabilidades

• Complete initial and ongoing patient/family assessments and plan of care.
• Administer medications and treatments as prescribed.
• Develop and revise care plans with patient and family involvement.
• Coordinate with physician and interdisciplinary team.
• Provide patient/family education and self-care instructions.
• Supervise hospice aides and participate in on-call duties.
• Maintain accurate clinical notes and ensure safety.
