Job summary

Amedisys, part of the Optum family, is hiring a Hospice Liaison/Hospice Sales Rep to educate providers and drive referrals. You will build relationships with hospitals, skilled nursing facilities, and physicians, coordinate admissions, and maintain records in CRM.

This role requires traveling 50%+ within the territory and delivering compassionate, patient-first care. Eligible candidates will have experience in healthcare business development and a valid driver’s license with insurance.

Qualifications

• 1+ year of experience in healthcare business development/outreach.
• Reliable transportation with a valid driver’s license and liability insurance.

Responsibilities

• Educates health care professionals, patients, and families about home care services.
• Develops and maintains relationships with key referral sources to generate leads.
• Coordinates patient admissions with the team and completes referral to admission process.
• Conducts on-site visits to assess patient needs and barriers to care.
• Keeps accurate records of patient interactions and referrals in CRM.
• Attends community events to promote Amedisys.
• Travel 50% or more within the assigned territory.
